Determinants of the Survival Duration of Primary Medical Clinics in Seoul

Abstract

This study analyzes the determinants of survival among clinic-level medical institutions amid rising closure risks. Using data on 8,616 clinics established in Seoul between 2014 and 2023, it applies Kaplan-Meier survival analysis, Cox proportional hazards models, and frailty models. The results show that medical specialty type and the number of physicians constitute key internal survival factors, with dental clinics exhibiting lower closure risk and Korean medicine clinics showing higher risk. Clinics operated by two or more physicians demonstrate higher survival probabilities than single-physician practices. In competitive environments, a higher density of nearby clinics within a 500-meter radius increases closure risk, while proximity to general hospitals and greater hospital density also elevate survival risk. Locational factors indicate that clinics located in areas with lower aggregate income, larger land parcels, and higher land prices face lower closure risk. Among socio-economic factors, a higher proportion of the elderly population reduces closure risk, whereas higher loan interest rates at the time of establishment significantly increase it. Frailty model results reveal heterogeneity across opening and closure years, highlighting the role of macroeconomic conditions. Overall, the findings demonstrate that clinic survival is jointly determined by internal characteristics, locational and competitive environments, and broader economic conditions, underscoring the importance of location-based supply regulation in healthcare policy.

본 연구는 의원급 의료기관의 폐업 위험이 증가하는 상황에서 의료기관 생존에 영향을 미치는 요인을 분석한다. 2014년부터 2023년까지 서울에서 개원한 의원급 의료기관 8,616개소를 대상으로 카플란-마이어 생존분석, 콕스 비례위험모형, 프레일티 모형을 적용한다. 분석 결과, 진료과 유형과 의사 수는 핵심적인 내부 생존 요인으로 나타나며, 치과의원은 폐업 위험이 낮고 한의원은 높은 경향을 보인다. 의사 수가 2인 이상인 의료기관은 1인 운영기관보다 생존 가능성이 높다. 경쟁 환경에서는 반경 500m 내 의원급 의료기관 수가 많을수록 폐업 위험이 증가하고, 종합병원과의 근접성과 주변 병원 밀도 또한 생존 위험을 높인다. 입지 요인 측면에서는 종합 소득액이 낮고 대지면적이 넓으며 공시지가가 높은 지역일수록 폐업 위험이 낮다. 사회･경제적 요인 중에서는 고령 인구 비중이 높을수록 폐업 위험이 감소하는 반면, 개원 시점의 대출금리는 폐업 위험을 유의하게 증가시킨다. 프레일티 분석은 개원 및 폐업 시점별 이질성을 확인하며, 거시적 경제 환경이 의료기관 생존에 큰 영향을 미침을 보여준다. 본 연구는 의원급 의료기관의 생존이 내부 특성뿐 아니라 입지, 경쟁 구조, 거시경제 요인에 의해 복합적으로 결정됨을 밝히며, 의료기관 개설 및 지역 보건정책에서 입지 기반의 공급 조정 필요성을 시사한다.
