Locational and Competitive Determinants of Survival Duration among Primary-Level Medical Clinics in Seoul, South Korea

Abstract

This study examines the effects of locational and competitive factors on the survival duration of primary-level medical clinics opened in Seoul between 2014 and 2023 (N = 8,616). In the absence of a clearly differentiated healthcare delivery system in Korea, clinics, hospitals, and tertiary hospitals operate within the same competitive market, where competitive density—particularly in metropolitan areas—structurally shapes organizational survival. Unlike prior studies that focus on hospital-level institutions or administrative-district indicators, this research quantifies micro-spatial competition at a 500-meter radius, centering on primary-level clinics. Clinic closure is defined as the event of interest, and survival data are analyzed using Kaplan–Meier estimation, Cox proportional hazards models, shared frailty models with a gamma distribution, and restricted mean survival time (RMST). The findings show that spatial competition intensity and locational characteristics significantly influence survival duration. Moreover, unobserved heterogeneity across medical specialties is statistically confirmed. By integrating market-based and spatial structural perspectives, this study advances the literature on healthcare organization survival through a micro-level analytical framework.

본 연구는 2014년부터 2023년까지 서울에서 개원한 의원급 의료기관 8,616개소를 대상으로 입지 및 경쟁 요인이 생존기간에 미치는 영향을 분석한다. 한국의 의료전달체계가 기능적으로 명확히 분화되지 않은 상황에서 의원･병원･종합병원은 동일 시장 내 경쟁에 노출되어 있으며, 특히 대도시에서는 경쟁 밀집도가 의료기관의 존속에 구조적 영향을 미친다. 기존 연구가 병원급 의료기관이나 행정구역 단위 지표에 집중한 것과 달리, 본 연구는 의원급 의료기관을 중심으로 반경 500m 단위의 미시적 공간 경쟁을 계량화하고, 폐업을 사건으로 정의한 생존 자료에 대해 카플란–마이어 추정, 콕스 비례위험모형, 감마 분포 기반 공유 프레일티 모형, 제한평균생존시간(RMST)을 적용한다. 실증 결과, 공간적 경쟁 강도와 입지 특성은 생존기간에 통계적으로 유의한 영향을 미치며, 진료과별 비관측 이질성 또한 확인된다. 본 연구는 의원급 의료기관의 존속을 시장 기반 요인과 공간 구조의 관점에서 규명함으로써 의료기관 생존 연구의 분석 단위를 미시적 수준으로 확장한 것에 그 의의가 있다.
